Vaccination is free. Bring picture ID with your birthday or you may self-attest at the site. No other documents are required. No questions about immigration status.
No appointment needed, but you may make one by calling 877-829-4692 and asking for the "mobile vaccine clinic" or online at vax4nyc.nyc.gov
Pfizer Vaccine will be available to anyone 12 years and older. The J&J vaccine will also be available at some sites.
Young people 12-15, must be accompanied by a parent or guardian.
All 16- and 17-year-olds can bring written consent, or their parents/guardians can provide verbal consent over the phone.
Interpreters who speak multiple languages will be available on-site to assist, if needed.

Anyone vaccinated at a NYC-run site (as opposed to a NY State run site, drugstore, or healthcare practice) is eligible for a $100 gift card. At-home vaccinations are included in this offer.
NYC restaurants, gyms, and entertainment venues now require customers to show proof of vaccination. To download the NY State Excelsior Pass to your mobile phone, go to: https://epass.ny.gov/home
