NHS Brooklyn counsels clients and make referrals with regard to:

  • Applying for affordable housing lotteries using Housing Connect

  • Eviction proceedings

  • Tenant rights

  • Succession rights

  • Legal assistance

  • Resources to address arrears

  • Rent Freeze for People with Disabilities: First application, renewal, appeal

  • Rent Freeze for Seniors: First application, renewal, appeal


To request an appointment. Please gather the documents listed below. (You may download the list below here.) Then call 718-469-4679 or click here to submit a request for an appointment with a

counselor. Be sure to bring your documents with you to your appointment.

Documents you must bring to an appointment with a Tenant Support Services counselor:


  • Current Lease Agreement

  • Income Information: Paystubs – Bring 4 if paid weekly, bring 2 if paid every two weeks; Also bring Social Security Award Letter, Pension, child support recent payment history, etc.

  • Federal Income Tax Returns for the past 2 years (Include all attachments and 1040s and W2’s)

  • Bank Statements (Two most current, all pages even if blank)

  • Current Utility Bill (Con Ed & National Grid)

  • Identification

  • Legal documents served (If any)
